Connect Me blood pressure service
This is a new service for patients who have their own home blood pressure monitors. It sends text messages, e-mails or phone messages to remind you to check your blood pressure and send the reading to the practice. If you would be interested in using this service, please speak to a member of our reception team.
Blood pressure readings done at home are more reliable than those done in the health centre, when you may be stressed or anxious. The system generates reports based on your blood pressure readings. These can be used to help diagnose high blood pressure, or to make sure that your blood pressure treatment is working well.
